Order Your Pets Repeat Prescription

Clients of Axe Valley Veterinary Surgeries can order any repeat prescription by completing this form. This service can only be used by clients of Axe Valley Veterinary Practice and is for pets who are on ongoing medication.  By entering your Axe Valley Pet ID we will be able to process your request more efficiently. If you don't know it just email us or ask at reception where we will be happy to help.

To ensure your pets are receiving the best possible treatment it is our strict policy that your pets is examined by us at least once every six months.

As soon as we receive your request, we will email you to confirm collection from your designated surgery.
